Disease Information

โ„น๏ธ What is Smoldering Multiple Myeloma (SMM)?

Smoldering Multiple Myeloma (SMM) sits between MGUS and active myeloma on the plasma cell disorder spectrum. Patients have a higher M-protein level (โ‰ฅ3 g/dL) or bone marrow plasma cell percentage (10-59%) compared to MGUS, but do not yet have the organ damage (CRAB criteria) that defines active myeloma.\n\nSMM carries a much higher progression risk than MGUS โ€” approximately 10% per year for the first 5 years. Risk stratification using the Mayo 20/2/20 model helps identify high-risk patients who may benefit from early intervention. The decision to treat versus monitor is one of the most active debates in myeloma, with clinical trials testing whether early targeted therapy can intercept progression and improve long-term outcomes.

๐Ÿ’Š Primary Treatment Approaches

๐Ÿ”ฌ Understanding Smoldering Myeloma

Smoldering Multiple Myeloma sits between MGUS and active myeloma on the plasma cell disorder spectrum. Like MGUS, SMM does not cause organ damage โ€” but unlike MGUS, the abnormal plasma cell burden is significantly higher, and the risk of progression is much greater, especially in the first 5 years.

The Plasma Cell Disorder Spectrum

Figure 1: The spectrum of plasma cell disorders. SMM is a middle stage between pre-malignant MGUS and active Multiple Myeloma.

๐Ÿ“Š Diagnostic Criteria

SMM requires either of:

  • โ€ข Serum M-protein โ‰ฅ3 g/dL
  • โ€ข Bone marrow plasma cells 10-59%
  • โ€ข Urine M-protein โ‰ฅ500 mg/24hr

AND: No myeloma-defining events (no CRAB, no biomarkers of malignancy)

๐Ÿ“ˆ Progression Risk

  • โ€ข ~10% per year for the first 5 years
  • โ€ข ~3% per year for years 5-10
  • โ€ข ~1% per year after 10 years
  • โ€ข ~50% progress within 5 years โ€” much higher than MGUS
  • โ€ข High-risk SMM: can progress in months

๐Ÿ“Š Risk Stratification โ€” Who Will Progress?

Not all SMM is equal. Identifying high-risk SMM โ€” patients likely to progress within 2 years โ€” is critical because early intervention may improve outcomescompared to waiting for organ damage.

๐Ÿ”ฌ The Mayo 20/2/20 Model

Three risk factors, each worth 1 point:

  • โ€ข Bone marrow plasma cells โ‰ฅ20%
  • โ€ข Serum M-protein โ‰ฅ2 g/dL
  • โ€ข Free light chain ratio โ‰ฅ20 (involved/uninvolved)
RiskFactors2yr Progression
Low0 factors~10%
Intermediate1 factor~26%
High2-3 factors~47-81%

๐Ÿงฌ Genomic Risk Features

  • โ€ข t(4;14), t(14;16), del(17p) โ€” high-risk cytogenetics even in SMM
  • โ€ข Circulating tumor cells in blood โ€” strong predictor of early progression
  • โ€ข Evolving M-protein โ€” progressive increase over serial measurements
  • โ€ข MYC rearrangements โ€” associated with rapid progression
  • โ€ข Whole-body MRI / PET-CT โ€” focal lesions may reclassify as active myeloma

๐Ÿ’Š The Early Treatment Debate

One of the most active debates in myeloma: should high-risk SMM patients receive early treatment before organ damage occurs?

โœ… Evidence FOR Early Treatment

  • โ€ข QuiRedex trial: lenalidomide + dexamethasone in high-risk SMM delayed progression (median not reached vs 23 months) and showed overall survival benefit
  • โ€ข ECOG E3A06: single-agent lenalidomide delayed progression in intermediate/high-risk SMM
  • โ€ข Preventing irreversible organ damage (renal failure, bone fractures)
  • โ€ข Earlier intervention when tumor burden is low and clone is less complex

โš ๏ธ Arguments for Observation

  • โ€ข Not all SMM progresses โ€” some patients remain stable for decades
  • โ€ข Treatment side effects in patients who may not need it
  • โ€ข Risk of developing drug-resistant clones before needed
  • โ€ข NCCN guidelines currently recommend observation with consideration of clinical trial enrollment for high-risk SMM

๐Ÿ”ฎ Current Practice & Emerging Data

The field is actively evolving. The AQUILA trial (daratumumab in high-risk SMM) and ITHACA trial (isatuximab-based) are testing whether modern immunotherapy can safely intercept progression. Many expert centers now offer treatment on clinical trials for high-risk SMM. Close monitoring every 3-4 months remains standard for all other SMM patients, with treatment initiated at progression to active myeloma (CRAB or myeloma-defining events).
