โ„น๏ธ What is Mantle Cell Lymphoma (MCL)?

Mantle Cell Lymphoma (MCL) arises from CD5+ B-cells in the mantle zone of lymph nodes. It is defined by the t(11;14) translocation, leading to cyclin D1 overexpression and dysregulated cell cycle progression. MCL accounts for about 5โ€“7% of non-Hodgkin lymphomas, predominantly affecting older men. MCL encompasses a wide spectrum: a subset of patients has the indolent, non-nodal leukemic form (SOX11-negative) that can be safely observed for years, while the majority have nodal MCL requiring prompt treatment. Blastoid and pleomorphic variants carry a poor prognosis. For younger, fit patients, intensive induction (NORDIC protocol or HyperCVAD) followed by autologous transplant is standard. For older or unfit patients, less intensive approaches like bendamustine-rituximab are used. BTK inhibitors (ibrutinib, acalabrutinib, zanubrutinib) are approved for relapsed MCL and are increasingly used upfront.

๐Ÿ’Š Primary Treatment Approaches

MCL treatment is selected by patient fitness and disease biology โ€” from intensive induction + transplant for younger patients to BTK inhibitors and targeted agents:

๐Ÿ”ฌ Disease Biology

Mantle Cell Lymphoma arises from CD5+ B-cells in the mantle zone surrounding germinal centers. It is defined by the t(11;14)(q13;q32) translocation in nearly all cases, which places the CCND1 gene (encoding cyclin D1) under control of the immunoglobulin heavy chain promoter, driving uncontrolled cell-cycle progression.

Classic Nodal MCL

The majority of MCL presents with lymph node involvement, splenomegaly, and circulating cells. SOX11 transcription factor is positive, confirming lymph node origin. Requires systemic treatment at diagnosis in most cases.

Leukemic Non-Nodal MCL

A biologically distinct subset (~10โ€“15%) that is SOX11-negative with mutations in IGHV genes. Presents as leukemia without significant lymphadenopathy. Often indolent โ€” watch-and-wait is appropriate for asymptomatic patients.

โš ๏ธ Blastoid & Pleomorphic Variants

~10โ€“15% of MCL cases exhibit blastoid morphology (highly proliferative cells resembling lymphoblasts) or pleomorphic morphology. These variants are associated with TP53 mutations, very high Ki-67, and significantly poorer prognosis. Standard regimens are often insufficient; clinical trials and experimental approaches are generally recommended.

๐Ÿ“Š MIPI โ€” Mantle Cell Lymphoma International Prognostic Index

The MIPI incorporates age, performance status, LDH, and WBC to stratify MCL into three risk groups. Ki-67 proliferation index strongly augments prognostication (biological MIPI).

Low Risk
>5 years
Median OS
Intermediate Risk
~4 years
Median OS
High Risk
~1.8 years
Median OS

Ki-67 โ‰ฅ30% doubles risk within each MIPI group. TP53 mutations confer ultra-high-risk biology regardless of MIPI score.

๐Ÿ’Š Treatment Approaches

Younger / Fit Patients

Intensive Induction + Auto SCT

Younger (typically <65), fit patients โ€” standard approach

NORDIC MCL2 / MCL-0208 protocols: R-MAXI-CHOP alternating with R-DHAP induction followed by autologous stem cell transplant (SCT) with BEAM conditioning. Rituximab maintenance post-SCT further prolongs remission. Ibrutinib addition to induction is under study.

HyperCVAD/MA + Rituximab

Alternate intensive induction (US centers)

Hyperfractionated cyclophosphamide, vincristine, doxorubicin, dexamethasone alternating with high-dose methotrexate and cytarabine. High CR rates but significant toxicity; generally recommended when autologous SCT is planned.

Older / Less-Fit Patients

Bendamustine-Rituximab (BR)

Preferred less-intensive induction

Superior PFS and OS vs R-CHOP in older MCL patients (BRIGHT trial). Often followed by rituximab maintenance. An excellent option for patients who are not transplant candidates.

Relapsed / Refractory Disease

BTK Inhibitors (Covalent)Ibrutinib (Imbruvica), Acalabrutinib (Calquence), Zanubrutinib (Brukinsa)

Standard relapsed MCL; all approved indications

Covalent BTK inhibitors achieve 70โ€“80% ORR in relapsed MCL with durable responses. Zanubrutinib demonstrated superior PFS vs ibrutinib (ALPINE trial) with a better toxicity profile (less atrial fibrillation). All 3 are approved for relapsed MCL.

Pirtobrutinib (Jaypirca)Non-covalent (reversible) BTKi โ€” FDA approved 2023

After at least 2 prior lines including a BTKi

Pirtobrutinib is a non-covalent BTK inhibitor that retains activity in patients who have acquired resistance mutations (C481S) to covalent BTKis. BRUIN trial: 57% ORR in BTKi-pretreated MCL. A critical option for patients who progress on ibrutinib/acalabrutinib/zanubrutinib.
