Disease Information

โ„น๏ธ What is Hodgkin Lymphoma (HL)?

Hodgkin Lymphoma (HL) is a cancer of B-lymphocytes defined by the presence of Reed-Sternberg (RS) cells โ€” large, distinctive cells that are the hallmark of this disease. HL accounts for about 10% of all lymphomas and is notable for its bimodal age distribution, peaking in young adults (15โ€“35) and again after age 55. HL is divided into Classic Hodgkin Lymphoma (cHL โ€” ~95% of cases, comprising nodular sclerosis, mixed cellularity, lymphocyte-rich, and lymphocyte-depleted subtypes) and Nodular Lymphocyte-Predominant HL (NLPHL). The disease spreads in an orderly, contiguous fashion through lymph node groups. PET-CT-guided, response-adapted therapy has allowed many patients to reduce treatment intensity while maintaining high cure rates. The major long-term concern is late treatment toxicity (heart disease, secondary cancers) after cure.

๐Ÿ’Š Primary Treatment Approaches

Hodgkin Lymphoma treatment is highly effective. PET-CT guides response-adapted therapy โ€” allowing de-escalation in excellent responders to reduce long-term toxicity:

๐Ÿ”ฌ The Biology of Hodgkin Lymphoma

Reed-Sternberg Cells

Hodgkin Lymphoma is defined by the presence of Reed-Sternberg (RS) cells โ€” large, bi-nucleated lymphocytes with prominent "owl-eye" nucleoli that are the hallmark of this disease. RS cells are actually rare within the tumor (<1% of cells); the surrounding inflammatory infiltrate of normal immune cells creates the bulk of the tumor mass.

Lymph Node Architecture

Unlike most non-Hodgkin lymphomas, Hodgkin Lymphoma spreads in an orderly, contiguous fashion through adjacent lymph node groups. This predictable spread pattern is why radiation therapy was historically effective and why PET-CT staging is so valuable for guiding treatment decisions.

Classic HL Subtypes

Nodular Sclerosis (NSHL)~70%

Most common subtype; often involves mediastinal nodes in young women. Characterized by collagen bands dividing lymph node into nodules.

Mixed Cellularity (MCHL)~20%

Associated with EBV in ~75% of cases; more common in older patients. Strong inflammatory background with eosinophils and plasma cells.

Lymphocyte-Rich (LRHL)~5%

Excellent prognosis. Many lymphocytes surround rare RS cells. Often mistaken for NLPHL.

Lymphocyte-Depleted (LDHL)~1%

Rare, aggressive subtype. More often EBV-positive; higher rates in immunocompromised patients and older adults.

Nodular Lymphocyte-Predominant HL (NLPHL)

NLPHL is a distinct entity (~5% of HL) characterized by popcorn cells (lymphocytic and histiocytic cells) rather than classic RS cells. It is CD20-positive (unlike classic HL), generally indolent, and has an excellent prognosis. Rituximab monotherapy is often used in early-stage NLPHL.

๐Ÿ’Š Treatment Approaches

Hodgkin Lymphoma treatment is highly effective. PET-CT scans after 2 cycles ("interim PET") guide response-adapted therapy โ€” allowing escalation in poor responders and de-escalation (less treatment) in excellent responders, reducing long-term toxicity.

ABVDAdriamycin (doxorubicin), Bleomycin, Vinblastine, Dacarbazine

Standard first-line for most patients

ABVD has been the global standard for >40 years. Given every 2 weeks; typically 2โ€“6 cycles depending on stage. Bleomycin-related pulmonary toxicity is monitored carefully.

BV-AVDBrentuximab Vedotin + AVD (no bleomycin)

Preferred for advanced-stage (III/IV) classic HL

Brentuximab vedotin targets CD30 (expressed on RS cells) coupled to a cytotoxic agent (MMAE). The ECHELON-1 trial showed superior PFS vs ABVD without bleomycin pulmonary toxicity. Now a preferred option for stage III/IV.

eBEACOPPEscalated Bleomycin, Etoposide, Adriamycin, Cyclophosphamide, Oncovin, Procarbazine, Prednisone

Intensive regimen for very high-risk advanced stage HL (European centers)

Higher complete remission rates than ABVD for high-risk advanced HL, but greater toxicity (infections, secondary malignancies, infertility). More commonly used in European centers; less so in the US.

PD-1 Inhibitors (Salvage/Relapsed)Pembrolizumab, Nivolumab

Relapsed or refractory classic HL

Classic HL exhibits high expression of PD-L1 on RS cells due to 9p24.1 amplification, making it exquisitely sensitive to PD-1 checkpoint inhibitors. Response rates of 65โ€“87% reported with pembrolizumab and nivolumab in multiply relapsed patients.

๐Ÿ“Š Prognosis & Survivorship

>85%
Overall 5-year survival
All stages combined
>95%
Early-stage cure rate
Stage Iโ€“II favorable
~75%
5-year survival
Stage IIIโ€“IV

Late Treatment Effects

Because many HL patients are cured young, long-term surveillance for treatment-related late effects is essential. Major concerns include:

  • โ€ข Cardiovascular disease โ€” elevated risk years after chest radiation or anthracycline chemotherapy
  • โ€ข Secondary malignancies โ€” particularly breast cancer in women treated with chest radiation under age 30
  • โ€ข Pulmonary toxicity โ€” bleomycin-related lung damage (managed by monitoring and avoiding bleomycin continuation)
  • โ€ข Hypothyroidism โ€” common after neck/mediastinal radiation
  • โ€ข Infertility โ€” a concern with alkylating agent-containing regimens; fertility preservation should be discussed before treatment

๐Ÿ“‹ International Prognostic Score (IPS)

The IPS is used for advanced-stage (III/IV) HL. Each of the following factors present at diagnosis scores 1 point; higher scores predict lower 5-year freedom from progression:

  • โ€ขSerum albumin < 4 g/dL
  • โ€ขHemoglobin < 10.5 g/dL
  • โ€ขMale sex
  • โ€ขAge โ‰ฅ 45
  • โ€ขStage IV disease
  • โ€ขLeukocytosis (WBC โ‰ฅ 15,000/ฮผL)
  • โ€ขLymphopenia (lymphocytes < 600/ฮผL or < 8% of WBC)

IPS 0โ€“1 = favorable (85%+ 5-year PFS); IPS โ‰ฅ4 = poor-risk (~42% 5-year PFS with ABVD; improved with BV-AVD).
