Disease Information

โ„น๏ธ What is Diffuse Large B-Cell Lymphoma (DLBCL)?

Diffuse Large B-Cell Lymphoma (DLBCL) accounts for approximately 25โ€“30% of all non-Hodgkin lymphomas. It is an aggressive malignancy that requires prompt treatment, yet about 60โ€“70% of patients achieve durable remission with first-line R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisone). DLBCL is biologically heterogeneous. The two major cell-of-origin subtypes โ€” Germinal Center B-cell (GCB) and Activated B-cell (ABC) โ€” have distinct molecular drivers and outcomes with standard therapy. High-risk features include double-hit/triple-hit lymphoma (concurrent MYC rearrangement with BCL2/BCL6), which may warrant intensified regimens. For relapsed/refractory disease, CAR T-cell therapies (axicabtagene ciloleucel, lisocabtagene maraleucel) and bispecific antibodies (glofitamab, epcoritamab) have dramatically improved outcomes.

๐Ÿ’Š Primary Treatment Approaches

DLBCL requires prompt treatment. R-CHOP or Pola-R-CHP is standard first-line; CAR T-cell and bispecific antibodies are transforming outcomes in relapsed disease:

๐Ÿ”ฌ Disease Biology

DLBCL is a cancer of mature B-lymphocytes in which large abnormal B-cells diffusely replace normal lymph node architecture. It is typically fast-growing โ€” nodes can double in days to weeks โ€” requiring prompt diagnosis and treatment.

Cell of Origin Subtypes

GCB

Germinal Center B-cell

~50% of cases. Arises from germinal center B-cells. Often carries BCL2 translocations and EZH2 mutations. Generally associated with a more favorable prognosis vs ABC with standard R-CHOP. Includes double/triple-hit lymphomas.

ABC

Activated B-cell

~30% of cases. Arises from post-germinal center B-cells. Driven by NF-ฮบB pathway activation; often carries MYD88 and CD79B mutations. Historically worse outcomes vs GCB with R-CHOP; newer regimens may close this gap.

โš ๏ธ Double-Hit & Triple-Hit Lymphoma (High-Grade B-Cell Lymphoma)

Approximately 5โ€“10% of DLBCL cases harbor concurrent MYC rearrangement plus BCL2 and/or BCL6 rearrangement โ€” termed double-hit (DHL) or triple-hit (THL) lymphoma. These are now classified as a separate entity (High-Grade B-Cell Lymphoma with MYC and BCL2/BCL6 rearrangements) and carry a significantly poorer prognosis with standard R-CHOP. Intensified regimens (DA-EPOCH-R, R-CODOX-M/IVAC) are commonly used.

๐Ÿ“Š International Prognostic Index (IPI)

The IPI predicts prognosis in DLBCL. Each factor below scores 1 point (max = 5):

โ€ขAge > 60
โ€ขSerum LDH > normal
โ€ขECOG Performance Status โ‰ฅ 2
โ€ขAnn Arbor Stage III or IV

High-Int (3)

High (4โ€“5)

5-yr OS

Approximate 5-year overall survival with R-CHOP. Outcomes continue to improve with newer agents.

๐Ÿ’Š Treatment Approaches

R-CHOPRituximab + Cyclophosphamide, Doxorubicin, Vincristine, Prednisone

Standard first-line for most DLBCL patients

6 cycles given every 21 days. Achieves durable remission in ~60โ€“70% of patients. CHOP20 (every 2 weeks with growth factor support) did not improve outcomes over standard R-CHOP-21 in most patients.

Pola-R-CHPPolatuzumab Vedotin + Rituximab + CHP (no vincristine)

Preferred for high-IPI (โ‰ฅ2) DLBCL in first line

POLARIX trial: Pola-R-CHP significantly improved PFS vs R-CHOP in IPI โ‰ฅ2 patients without additional toxicity. Polatuzumab vedotin is an anti-CD79b antibody-drug conjugate (ADC). Now a preferred first-line option for intermediate/high-risk DLBCL.

CAR T-Cell TherapyAxi-cel (Yescarta), Liso-cel (Breyanzi)

2nd line for transplant-ineligible; 3rd line+

ZUMA-7 (axi-cel) and TRANSFORM (liso-cel) trials demonstrated that CAR T is superior to salvage chemo + autologous SCT in second-line LBCL. Both axicabtagene ciloleucel and lisocabtagene maraleucel are now FDA-approved for early second-line (primary refractory or relapse <12 months).

Bispecific AntibodiesGlofitamab (Columvi), Epcoritamab (Epkinly)

Relapsed/refractory 3rd line+

These "off-the-shelf" T-cell engagers simultaneously bind CD20 on tumor cells and CD3 on T-cells, directing cytotoxic killing. Glofitamab (fixed duration, 12 cycles) and epcoritamab (subcutaneous injection) both have ~40% complete response rates in heavily pre-treated DLBCL. CRS management is key.
